In 2025, the Belgian government withheld nearly €15 million from the administrative allowances paid to health insurance funds because the performance targets set for them were not fully achieved. The figures come from data provided by Belgium’s National Institute for Health and Disability Insurance, as reported by De Tijd and HLN.
Since 2022, part of the funding received by health insurance funds has been linked to their performance. Up to 20% of their annual administrative allowances can depend on results against various performance criteria. These include the timely processing of reimbursements and benefits, internal controls aimed at preventing abuse, and efforts to support the reintegration of people on long-term sick leave.
The amount withheld has increased steadily, from €8.5 million in 2023 to €11.7 million in 2024 and €14.8 million in 2025. This represents an increase of nearly 75% in two years. Compared with the total administrative budget of more than €1.4 billion, however, the amount withheld remains relatively limited.
The figures are part of a broader move towards closer oversight of how health insurance funds carry out their statutory responsibilities. De Tijd had previously reported on increased supervision of the approximately €1.4 billion allocated each year to the administration of health insurance funds.
